There were instances reported on this forum in the past, which the defective search system doesn't enable me to find and revisit. And others reported in the meeja which I don't have retrospective access to. There's been enough 'smoke' that way to make people wary of getting their fingers burnt in the fire, and I for one aren't prepared to waste further time arguing about it here - particularly as the issue no longer affects me personally.
Re absolute perfection, yes I do think it should - just as, in my long personal experience, bank and credit card statements are, except in one instance when a handful of (readily detectable, and refunded without problem) fraudulent transactions appeared on a couple of credit card statements. If banks and other suppliers were as uneliable, inconsistent and unfair as TfL is with its fare charging systems I'd soon be changing my bank - and if I found they were all the same and couldn't be made to mend their ways, then I'd need a very good reason to carry on using banks at all.
